Diagnostic Test for Helicobacter Pylori infection by Histology and Rapid Urease (CLO) Test at Surin Hospital

Abstract
Background: H. pylori infection is the main etiologic factor for peptic ulcer disease, successful diagnosis and eradication of H. pylori infection has been shown to prevent complicated from peptic ulcer disease.
Objectives: The aim of this study was to evaluate the sensitivity and specificity value of histologic examination and rapid urease (CLO) test for diagnostic H. pylori infection and study the cost of diagnostic test.
Study Design: Retrospective study.
Methods: A retrospective review of patients undergoing esophago-gastro- duodenoscope (EGD) for diagnostic H. pylori infection by histologic examination and rapid urease (CLO) test between September 2008 and October 2009.
Result: Five hundred forty-four patients with a mean age of 60.34 years were included. Three hundred seventy patients(68%) were found to be positive for H. pylori. Three hundred twenty-six patients (88.1%) were found to be positive on histologic examination and one hundred sixty-four patients (44.3%) positive on rapid urease (CLO) test. The sensitivity of the rapid urease (CLO) test and histologic examination were 44.32% and 88.11% respectively. There was a statistically significant difference between the sensitivity found for rapid urease (CLO) test and histologic examination (p-value < 0.001).
Conclusion: Histologic examination seemed to be more sensitive than rapid urease (CLO) test in detecting H. pylori infection at Surin hospital.
