Statins and Cognitive Decline: A Double-Edged Sword or a Hope for Prevention?

Statins are cornerstone lipid-lowering medications for the prevention of cardiovascular disease. However, their impact on cognition remains a subject of ongoing debate, with evidence suggesting both potential dementia prevention and reports of statin-induced cognitive decline. This review article aims to explore the latest academic evidence by compiling data from observational studies, randomized controlled trials (RCTs), meta-analyses, and the most recent international clinical guidelines (2023-2025 A.D.). The findings reveal that research evidence remains contradictory; while observational studies and meta-analyses indicate a protective effect, large-scale RCTs show no clear association with either cognitive protection or harm. Mechanistically, the protective effects may derive from anti-inflammatory properties and vascular improvement, whereas potential adverse effects may relate to the reduction of brain cholesterol, particularly with lipophilic statins. In conclusion, the cardiovascular benefits of statins are supported by robust evidence and significantly outweigh the uncertain risks to cognition. This aligns with recent international guidelines, which affirm that the majority of evidence points toward a neutral or potentially protective effect. Although current evidence does not support the direct use of statins for dementia prevention, it strongly discourages drug discontinuation in patients with clear clinical indications. Therefore, pharmacists play a crucial role in providing patient counseling, fostering accurate understanding, and monitoring potential adverse events to ensure optimal therapeutic outcomes.
