Health Belief Model and Fall Prevention Behaviors among Older Adults: A systematic review

Abstract

This systematic review aimed to examine the role of the Health Belief Model (HBM) in fall-prevention behaviors among older adults using articles from ScienceDirect, CINAHL, PubMed, Google Scholar, Scopus, and ThaiJO, published from 2021-2025, using PICO-based keywords: HBM, fall prevention behaviors, and older adults. Research quality was assessed using Heller’s appraisal tool.

Results showed that 10 studies met the inclusion criteria, most of which were quasi-experimental studies involving older adults in communities, hospitals, and elderly clubs in Thailand and other countries such as South Korea, China, Brazil, and Vietnam. The studies evaluated the effectiveness of HBM-based programs on fall-prevention behaviors. Almost all studies reported positive outcomes, including improved knowledge, perceived severity, perceived susceptibility, and fall-prevention behaviors. Programs integrating additional components such as social supports, health literacy enhancement, and home environmental modification showed greater effectiveness. Experimental groups demonstrated significantly higher scores in health beliefs and preventive behaviors than those from the control groups (p<0.05-0.001). International findings also supported that HBM strengthens self-efficacy and reduces fear of falling among older adults.

It can be concluded that the applications of HBM, combined with social supports and multi-component interventions, is an effective approach for developing fall-prevention models for older adults and can be applied to community settings in Thailand.
