Task-Specific Training Methods for Balance and Ambulation in Stroke Patients' Rehabilitation

Abstract

Background and Objective: Balance impairment is a major problem among stroke patients and significantly increases the risk of falls. Task-oriented circuit training emphasizes repetitive practice of functional tasks relevant to daily activities and may enhance motor recovery. This study aimed to compare the effects of task-oriented circuit training on balance, gait ability, and activities of daily living in patients with hemiplegia.

Methods: This randomized controlled trial included 32 patients with stroke who were randomly assigned to an experimental group (n=16) and a control group (n=16). The experimental group received a task-oriented circuit training program, whereas the control group received conventional physical therapy. All interventions were administered over a
2-week period. Outcomes were assessed before and after the intervention using the Berg Balance Scale (BBS), the 10-Meter Walk Test (10MWT), and the Barthel Index. Baseline-adjusted analyses were performed to compare post-intervention outcomes between groups.

Results: After the intervention, the experimental group demonstrated significantly greater balance ability than the control group, as indicated by higher BBS scores (mean difference=7.34 points; 95%CI: 4.50-10.18; p<0.001). Gait speed measured by the 10MWT was also significantly better in the experimental group (median difference=0.045 m/s; 95%CI: 0.006-0.083; p=0.024). In addition, the experimental group achieved significantly higher Barthel Index scores compared with the control group (mean difference=2.89 points; 95%CI: 1.94-3.83; p<0.001).

Conclusion: Task-oriented circuit training is effective in improving balance, gait performance, and activities of daily living in patients with hemiplegia. This training approach can be recommended as an effective rehabilitation strategy for stroke patients in clinical practice.
