Appropriateness of Blood Preparation for Patients Underwent Elective Surgery in Mahasarakham Hospital

Background: Inappropriate blood preparation leads to a costly waste of resources and increases blood bank workload. This study aimed to evaluate the blood utilization and cost effectiveness for patients underwent elective surgery in Mahasarakham Hospital classified by department and type of surgery. Methods: A retrospective review of preoperative blood preparation for elective surgery was done at Mahasarakham Hospital for one year period (October 2022 to September 2023). The appropriate of blood preparation was indicated by crossmatch to transfusion ratio (C/T ratio), transfusion probability (%T) and transfusion index (Ti). Result: There were 1,987 patients underwent surgery, 305 patients (15.4%) had routine crossmatching and 1,682 patients (84.7%) had type and screen method. Overall blood preparation was inappropriate (C/T ratio = 2.48, %T = 32.13, and Ti = 0.48) but due to the increase in type and screen method, this can save the cost of 236,550 baht, and the cost of preparing non transfused blood was wasted 115,560 baht. Conclusion: Overall blood preparation was inappropriate. However, it has been found that type and screen method has increased in proportion. Maximal surgical blood order schedule (MSBOS) should be used together with type and screen method to be used in the type of surgery that was inappropriate blood preparation.
